About the Role:
As a Clinic Care Coordinator, you will play a vital role in supporting our patients by:
Connecting them with local mental health and social support resources
Collaborating with our medical team to ensure holistic patient care
Advocating for patients and removing barriers to care
Developing creative solutions to support patients’ mental and physical well-being

Qualifications:
We welcome applicants that are Licensed Clinical Social Workers, Master of Social Work, or applicants with a bachelor’s degree of varying backgrounds.
