Behavioral Health Care Manager

Position Summary:

We are seeking a compassionate and motivated Behavioral Health Care Manager to join our clinic team. Key Responsibilities:

Collaborate closely with primary care providers, psychiatric consultants, and other care team members to develop and implement care plans for patients with behavioral health conditions.
Provide systematic case review and consultation with psychiatric providers on assigned caseloads.
Track patient progress using evidence-based tools (e.g., PHQ-9, GAD-7) and registries to ensure measurable improvement in symptoms and functioning.
Conduct regular patient follow-ups to monitor adherence to treatment plans, offer problem-solving support, and adjust care as needed.
Coordinate referrals to specialty mental health providers or community-based services when appropriate.
Maintain clear, timely, and accurate documentation within the electronic health record (EHR).
Participate in team huddles, case conferences, and quality improvement initiatives to support an integrated, whole-person approach to care.
Educate and support patients and families around mental health conditions and treatment options.
Qualifications:

Experience:

Experience working in a behavioral health or integrated care setting is a plus.
Experience with care coordination, patient engagement, and evidence-based interventions (e.g., brief CBT, motivational interviewing) preferred.
Skills:

Strong interpersonal, communication, and organizational skills.
Ability to work effectively within an interdisciplinary team.
Familiarity with behavioral health screening tools and outcome tracking.
Proficiency with electronic health records and documentation practices.
Preferred Qualifications:

Prior experience in a primary care, rural health, or federally qualified health center (FQHC) setting.
