Community Health Assessment
The Community Health Improvement Plan was done in conjunction with the Mineral County Health Department. This data was gathered by a group of volunteers going door-to-door to ask a series of questions to find out the concerns are of Mineral County residents. The Mineral County Health Department and Mineral Community Hospital are working together to implement a plan of action to address these health concerns.
